What MUST BE entered in the form?
- Requested category of equipment
- Description of the event the equipment will be used for, including relevant technical information
- Period of rental
- Details of the person to be included in the contract: name, date of birth, UČO, position at the Faculty of Arts
- Place of the event
- Purpose of the recording and expected use of the recording in the future
- If you have any additional information about your request, please, include it as well.
