Exit Process Form (Dean's Office SCI)
Maybe, you are in doubt with these forms and you prefer sending an e-mail or calling someone personally. Still, if you do not know who to contact, this form is the fastest method to deliver your inquiry to the most qualified person. At least give it a try.

Well in advance of termination, upload all your work documents and emails to the departmental share disc on the O: drive or to the Sharepoint share. If you are unclear or have any difficulties, please contact the faculty ICT department (it@sci.muni.cz) and your head of department by email as soon as possible.
